We require written consent to share information with external services.

Where there is a serious concern for a person’s safety or wellbeing, we are legally required to report this to the appropriate service and do not require consent.

Written consent is only valid for a maximum of 12 months.

You have a right to access the information held by Azure and to request any information be corrected.
