Our Occupational Therapist
My name is Jayde Lambert
I am an Occupational Therapist with over 10 years’ experience working across hospital, rehabilitation and community settings in Australia and overseas. After returning to Australia in 2016, I found my passion in community-based occupational therapy, supporting people in their own homes and everyday environments.
​
Over the past two and a half years, I have been building Azure Occupational Therapy as a values-driven, adult-focused community OT service. My work primarily involves supporting adults with disability through the NDIS, with a strong focus on functional assessment, assistive technology, home modifications, and housing-related assessments. I work closely with participants, families, and other professionals to ensure supports are practical, appropriate, and aligned with each person’s circumstances.

Alongside clinical work, I have developed clear systems, resources, and clinical frameworks to support ethical, consistent, and high-quality practice. Azure has been intentionally designed as a small practice so I can remain closely involved in assessments, clinical reasoning, and service delivery.
