
OCCUPATIONAL THERAPY
Occupational therapy supports people to participate in everyday activities that are meaningful to them, and to complete these activities as safely and independently as possible.

At Azure Occupational Therapy, I work with you to understand the activities you would like to do, return to, or find easier. An assessment is completed to explore your strengths, abilities, environment, equipment, experience, and both formal and informal supports. Together, we identify any challenges impacting task completion and consider practical strategies to support your goals.

Following assessment, a plan may be developed in collaboration with you and, where appropriate, your supports or other services. This plan may include a range of occupational therapy interventions, such as:

Home modifications – recommendations for changes to the built environment

Equipment prescription / Assistive Technology (AT) – trialling and recommending equipment

Skill development – practising and learning everyday skills, sometimes supported by tailored resources

Functional assessments – including Functional Capacity Assessments (FCA), SIL and ILO assessments
-
Housing assessments – including SDA-related assessments and recommendations
-
Reports for funding bodies – such as the NDIS, State-Wide Equipment Program (SWEP), or Home Care Packages (HCP)

Plans are reviewed with you either at regular intervals or at the conclusion of a goal, depending on the length and nature of the service. This allows time to reflect on progress and adjust supports where needed.

Areas of Practice
Azure Occupational Therapy provides community-based occupational therapy services for adults. Areas of practice may include the following.
Functional Assessment and Planning
Assessment of everyday function, strengths, and support needs to inform planning and service delivery. This may include Functional Capacity Assessments and other functional or housing-related assessments for funding or support planning purposes.
Assessments may involve working with individuals who have high and complex functional support needs, requiring consideration of safety, environmental factors, communication, and the involvement of multiple supports or services. The focus remains on understanding function within real-world contexts and supporting informed, collaborative planning.

Assistive Technology (AT)
Assessment, trial, and recommendation of assistive technology to support mobility, safety, and participation in daily activities. This may include, for example:

Manual and power wheelchairs

Mobility scooters

Seating and positioning equipment

Beds and bedroom equipment, including profiling beds

Riser recliners and lift chairs

Daily living and personal care equipment

Home Modifications
Assessment and recommendations for home modifications to support safety and independence within the home environment. This may include:

Minor home modifications, such as grab rails, hand-held showers, and access improvements

Complex home modifications, including bathroom, kitchen, and access modifications that involve structural changes
Recommendations are developed in line with individual needs, relevant standards, and funding requirements, and may involve collaboration with builders and other professionals.
Housing and Living Assessments
Assessment and reporting to support decisions related to housing and living arrangements, including:

Supported Independent Living (SIL)

Individualised Living Options (ILO)

Supported Disability Accommodation (SDA)
These assessments consider functional needs, safety, and environmental factors.

Skill Development and Functional Participation
Support to build, maintain, or adapt everyday skills that enable meaningful participation in home and community life. This work focuses on practical strategies, environmental supports, and graded skill development, rather than reporting alone.

This may include supporting individuals with high and complex functional support needs, including situations where unmet needs, environmental factors, or communication challenges may contribute to behaviours of concern. Occupational therapy input focuses on understanding functional drivers, supporting participation, and working collaboratively with families and support teams to enable safer and more sustainable engagement in everyday activities.
Reports and Funding Applications
Preparation of occupational therapy reports to support access to funding and services, including reports for the NDIS, State-Wide Equipment Program (SWEP), and Home Care Packages (HCP).
